We make the care transition process simple and stress-free with personalized guidance every step of the way
Reach out to ElderPath via phone, email, or our online contact form. We'll schedule a free consultation to understand your situation, needs, and concerns. This is a no-pressure conversation where we listen and learn about your loved one.
What to expect: A compassionate conversation (30-60 minutes) where we discuss current challenges, care needs, budget considerations, and preferred locations.
We conduct a thorough assessment of your loved one's physical, cognitive, social, and emotional needs. We also review medical history, current medications, and any special requirements like memory care or skilled nursing.
What we evaluate: Activities of daily living (ADLs), mobility, memory/cognition, medical needs, social preferences, and financial resources including insurance and Medi-Cal eligibility.
Based on our assessment, we develop a personalized care plan and present suitable options. This might include assisted living, memory care, board & care homes, or skilled nursing facilities that match your loved one's needs and preferences.
Our recommendations include: Detailed information about each facility, costs, availability, services offered, and how well they align with your specific needs.
We coordinate and accompany you on tours of recommended facilities. We help you ask the right questions, observe key details, and compare options. You'll never feel rushed or pressured—we're there to support your decision-making.
Tour support includes: Pre-tour preparation, on-site guidance, post-tour debriefing, and comparison of options to help you make an informed choice.
Once you've selected a facility, we coordinate all aspects of the move. This includes paperwork assistance, move-in scheduling, medical record transfers, and ensuring a smooth transition. We remain available to address any concerns during the adjustment period.
Transition support includes: Admission coordination, communication with healthcare providers, move-in assistance, and follow-up to ensure satisfaction and proper care delivery.
Our relationship doesn't end at move-in. We follow up to ensure your loved one is settling in well and receiving appropriate care. If issues arise or needs change, we're here to advocate and help coordinate any necessary adjustments.
Continued support: Regular check-ins, advocacy if concerns arise, assistance with care plan adjustments, and support through any future transitions.
