Supporting Care Transitions with Clarity and Dignity

Care transitions—such as moving from skilled nursing to the community, or from assisted living to higher levels of support—can be overwhelming. ElderPath exists to help individuals and families understand their options, coordinate services, and move through these transitions safely and thoughtfully.

We focus on coordination, communication, and advocacy, not placement or referrals. Our role is to support informed decision-making and continuity of care while working alongside healthcare providers, care teams, and authorized representatives.

Navigating Publicly Funded Care Programs

ElderPath assists individuals and families in understanding and navigating publicly funded care programs, including Medi-Cal-related initiatives and community-based services.

We provide education, coordination support, and guidance while working alongside plan-approved and licensed providers.

What We Provide

Education and coordination support for Medi-Cal programs, Enhanced Care Management (ECM), CalAIM initiatives, and Assisted Living Waiver (ALW) programs.

Important Clarification

ElderPath does not provide Medi-Cal benefits directly, does not determine eligibility, and does not act on behalf of any managed care plan.

Decision-Making Authority

All care and housing decisions remain with the individual, family, and licensed providers. ElderPath does not make these decisions on behalf of clients.

We do not provide medical, clinical, or legal services unless otherwise specified by contract. Our role is to coordinate, educate, and support—not to direct or decide.
