The Right Path to Elder Care
For many adults, aging and memory changes can be a challenge. ElderPath helps older adults and families choose the right path to care. Clients meet with an Elderpath consultant, a professional in geriatrics to discuss resources and plan for the future. The ElderPath consultant provides extensive expertise in geriatric care particularly when there are memory or mood changes. The end result is a Care Recommendation Plan that supports your goals. Memory care coaching for caregivers is a specialty.

Family Mediation Services
ElderPath offers family mediation services when family members are at a crossroads regarding the memory care of an older adult. The mediator works with family members to come to a consensus around care needs.

Evening/Weekend availability
For your convenience, an ElderPath consultant is available evenings and weekends. The consultation can take place in your home or our Glastonbury office.

ElderPath is associated with Dr. Harry Morgan and The Center for Geriatric and Family Psychiatry in Glastonbury. We offer direct referral to this Center.

Consultants are sensitive to the needs of older adults in diverse cultures and in the LGBT community.

Consultations can be done in Spanish.

From Bob in Oregon:
Karen at ElderPath has been wonderfully helpful and thorough as she's put together an integrated system of care for a loved relative who lives in Connecticut, which is 3,000 miles away from me. She made her initial assessment of my uncle, then continued to make recommendations for ongoing
care and keep me informed via phone calls and emails. This level of commitment speaks to her
integrity, her ability to organize, and her level of caring as a human being. As a licensed clinical social worker in Oregon, I've worked
with others in my profession and I know the stresses involved in providing care for people in need and keeping their families involved and comforted. Karen has come through, and we're grateful. I strongly recommend her services.
